Voter's Registration Application Form
Who can register to vote:
- You must be at least 17 years of age to register;
- You must be a citizen of the United States;
- You must reside in Miami-Dade County. There is no length of time that you must live here before registering to vote.
Who cannot register to vote:
- Anyone who does not meet the above requirements;
- Anyone who has been declared "mentally incompetent" who has not had his/her rights restored.
- Anyone convicted of a felony who has not had his/her civil rights restored
How to register to vote:
- Registering to vote is a right. It is available to all persons who meet the requirements. In Miami-Dade County, there are many official agencies where you can register in person or get voter registration forms:
- Florida Driver License Bureau
- Miami-Dade County Public Library
- Any State public assistance agency
- Any agencies that provide assistance to disabled persons
- Armed Forces Recruiting Office
- If you need more information regarding the closest agency, or to obtain a form by mail, contact our office any weekday between 8 AM and 5 PM, at (305) 499-8363.
- When you register to vote, an actual residence address is required by law. If you provide any kind of mailing service address, PO box, or business address, the application will not be accepted, and can cause your registration to not be accepted, and can cause your registration to not be processed in time for an election. PO boxes and other business addresses can be accepted as a mailing address.
- You may register online using the Voter's Registration Application Form provided by the State of Florida Division of Elections. Type your information onto the form and then print, or you may first print out the form and then fill out the information by hand. When the form is completed, please sign it, and then send the signed form to:

Voter Registration Deadlines (Book Closings):
Registration closes 29 days prior to an upcoming election (Book Closing date). In order to be eligible to vote, your Voter's Registration Application Form must be received one of the following ways:
- Go in person to the Elections Department or any other official agency no later than the book closing date;
- Have it mailed and postmarked no later than the book closing date.
Deadline at registration sites:
- At certain locations applications will be kept and delivered to the Elections Department.
- Voter's Registration Application Forms must be turned in at the registration site at least 29 days before an election in order to meet the book closing deadline.
